A client’s attendance of a therapy session with Stephen Jacobs implies acceptance of the terms of service, privacy and consent described on this page. This step is mandatory to the provision of services.
A digital intake form will be provided to the client prior a first appointment. The intake form collects relevant personal background client information and asks the client to digitally sign the form and terms of services to confirm their consent. Stephen Jacobs will explain and review the consent form and terms of services at the beginning of a new client session, and answer any related questions.
In the event of a technical issue prevents the client to accept and sign the terms of services and consent form digitally, a paper form will be signed before provision of services.
Venues for Appointments
As of 1 December, 2023, Stephen Jacobs – EMDR Therapy consults at Suite 13, 594-596 Crown Street, Surry Hills. The hours of consultation are 11am to 6pm, no client support is provided outside of consultation hours.

2024 Fees / Cancellation Policy
The $190 (for 50 minutes) appointment fee is payable by Visa/Mastercard. If, for some reason you need to cancel or postpone the appointment, please provide at least a notice of 2 business days (which excludes weekends and holidays), otherwise, you will be charged the full fee.
For Tuesdays appointments, we require notification of cancellations by noon on the previous Friday. This gives us the time to contact clients on our waitlist and offer them your cancelled session. We appreciate your consideration in this matter, as will other clients who are on the waiting list.
In the event of 2 client cancellations in the course of a treatment cycle, we reserve the right to reschedule the remaining appointments.
As part of providing our service to you, we will collect and record personal information from you that is relevant to your current situation. This information will be a necessary part of the assessment and treatment. Psychotherapists may talk confidentially about clients as part of formal and informal peer supervision.
All personal information gathered during the provision of the service remain confidential and secure except when:
- It is subpoenaed by a court, or
- Failure to disclose the information would place you or another person at risk.
The terms above apply to all clients and cover the provision of psychotherapy services. It is the client responsibility to indicate whether they disagree with the above terms, and should discontinue attending the psychotherapy services until a reciprocal agreement has been reached.
The terms above are presented by Stephen Jacobs during the first session. Amendments to this document will be explained to the active clients.
